I am unable to accept any direct payment from new patients with Medicare. I do not accept insurance but I can provide you with an itemized receipt or superbill that your insurance company accepts for out-of-network benefits.Please Click Book Now for either a New or Follow-Up Appointment. If you have Medicare at all you may not book an appointment. I am unable to accept any direct payment from new patients with Medicare. I can only see residents of TN. All Telehealth New Visits must have the New Patient Packet with photo of drivers license (front and back) emailed to me before the visit (email at top of New Patient Packet). If you are a resident of TN but have out of state DL, I will need a recent (past 3 months) utility bill or lease or contract confirming you are a resident of TN. For in office appointments you can either download, print and fill out our New Patient Form to bring with you to your first appointment or arrive early and fill out in waiting room.
